WHAT IS HOUSING STABILIZATION?
Housing Stabilization Services (HSS) is a new Home and Community based Services (HCBS) that supports people with disabilities or seniors in finding a stable home. The Housing Stabilization consists of three services: Housing Consultation, Housing Transitioning, and Housing Sustaining.
THE PURPOSE OF THESE SERVICES IS TO:
Support an individual’s transition into housing
Increase long-term stability in housing in the community
Avoid future periods of homelessness or institutionalization
HOUSING CONSULTATION:
• Housing Consultation assist a person in developing a housing focused person-centered plan • Assist the person to access needed state plan services that support housing stability and provide referrals or information about other services. • This service is available to people on Medical Assistance who do not have a waiver case manager or targeted case management or a MSHO/MSC+ care coordinator involved to complete an assessment and a person-centered plan.
HOUSING TRANSITIONING:
• Developing a housing transitioning plan • Identifying and assisting in resolving barriers to assessing housing • Searching for stable home for the individual • Supporting the individual in applying for benefits to afford their housing • Contacting housing options for availability and information • Help with tenant screening and housing assessment
HOUSING SUSTAINING:
• Early intervention and spotting behaviors that will make a client lose a home • Training in being a good tenant, lease compliance, and management • Supporting the person to understand and maintain housing
To Qualify for Housing Stabilization Services, you must meet this following criteria:

- 18 years of age or older
- Receiving Medical Assistance(MA)
- Having a disability or a disabling condition Experiencing housing instability (Disabilities can include physical disabilities, mental illness, substance use disorder, and other conditions.)

• Professional Statement of Need, OR
• Medical Opinion Form, OR
• Proof of Recipient of SSI or SSDI, OR
• 65 Years Or Older
Assessment Type:
• Professional Statement of Need, OR
• MNChoices Assessment, OR
• Coordinated Entry, OR
• Long Term Care Consultation
Person Centered Plan Type:
• Housing Focused Person Centered Plan, OR
• Coordinated Services and Supports Plan (CSSP)
